First Therapy Appointment

In the first two to three sessions we will focus on your reasons for coming to therapy, your background, and goals for our work together. This early background information provides me with the context I need to be in a position to provide insight. Also, by the end of three sessions we’ll have a good idea of whether we will be a good fit in working together.

Prior to coming in, please read and sign the paperwork below. If you are unable to do so, we’ll use the first portion of your session for completing your paperwork (just keep in mind that this will shorten the amount of time we have to talk and you’ll be surprised how fast time flies!).

Fees

I am a Regence provider. If you would like to use your Regence insurance plan I will submit the paperwork and you will be responsible for your copay at each session.

For all other insurance plans I am an out-of-network provider. I will gladly submit your claim for seeing me as an out-of-network provider. Before our first session please contact your insurance company to find out your specific benefits. I recommend asking:

  • Your deductible amount and how much has already been met
  • The percentage at which sessions will be covered for an out-of-network provider
  • The number of sessions covered during a calendar year

I will assist in submitting your claim after our first session.  Couples therapy is generally not covered by insurance (though Employee Assistance Programs, EAP’s, often cover a number of sessions).

If you need to cancel or reschedule, I require at least a 24-hour notice or you will be charged the full session cost for that missed appointment (of course emergencies and other unforeseen events, such as inclement weather, will be taken into consideration). If you arrive late to our appointment, we will begin when you arrive and end at our regularly scheduled time.

I currently accept cash or check at the time of our session. Checks are payable to: Amber Tabares, Ph.D., PLLC
